Hong Kong’s leader on Tuesday announced an independent judge-led commission to investigate last week’s apartment fire that killed 151 people. Officials said the city’s worst fire in decades spread rapidly through an outdoor network of scaffolding that failed to meet fire-resistance standards and failed to prevent the flames from spreading.
