Troye Sivan is reclaiming the narrative after a TikTok doctor publicly dissected her appearance and suggested she get cosmetic procedures.
The singer-songwriter30-year-old wrote about aging, self-image and the pressure to stay forever young in a new substack post published on Wednesday, January 21, in which she opened up about the unsolicited plastic surgery advice she received before revealing that the doctor issued her a “thoughtful” apology.
“I constantly oscillate between feeling like I’m aging in a good way, getting ‘sexier’ over time, and then feeling like Gollum’s very close, pop-singing relative,” Sivan began. “So decrepit, somehow thin and fat at the same time. I’m 30. I’ve struggled with my body image for much of my life, as I’m sure most people have.”
Sivan addressed the constant feeling of falling short of an ever-shrinking ideal, adding: “I’m historically famously skinny, and I’m not THAT skinny anymore. I’m historically famously twinky (I’m still the image in the Google search results for ‘twink’) and I’m not THAT twink anymore. Wow.”

The first is, “I’m body positive at my core and I believe every body is beautiful,” she explained. “I’m thankful for mine (and yours, sexy), and that it allows me to do all the things I want, free of pain and illness. It’s also great to get old.”
Sivan said he heard these words of wisdom “from a person at the top of the fashion world,” citing the phrase, “stay ugly” while clarifying that it essentially meant “don’t screw up your face.”
In addition to this, the social media star said that he is proud of the recent efforts he has made to improve his overall well-being, including going to the gym and following a nutrition plan since December 2025.
“I’ve been getting bigger, still lean, but more muscular, defined and toned. It’s only been 6 weeks and sometimes I run my hands over my chest and it feels like it belongs to someone else,” she shared.
Sivan noted that, on the other hand, he has often criticized himself and also dabbled with the idea of tweaks and tweaks, although he noted that they can still be “dangerous” and turn out “bad” even after a massive investment. The star admitted she was previously quoted $3,000 during an eye fat transfer consultation.
“I don’t really want that frozen look, but I notice my ‘elevens’ starting to show even when I’m not squinting. I frown all the time because I have light blue eyes that are very sensitive to the sun,” she continued. “What good is money and modern medicine if not to fix all these flaws that this random plastic surgeon told me I have on an instagram feed?”
Sivan said the red carpet photos didn’t turn out to be desirable, confessing that “I’m too embarrassed to dress the way I think people who looks good on red carpets put.”
“Hearing this unsolicited medical advice given publicly by a doctor I don’t know pushed me towards point 2 for several days,” he continued. “My omniscient, eternally and deeply evil algorithm saw opportunity in this moment of vulnerability and pulled all the levers and dialed all the buttons to 1000. I watched video after video of deep facelift recoveries and commercials for unapproved GLP-1 medications which now come in pill form (I currently weigh 59kg/130lbs).
Sivan concluded his post with a hopeful message about the future, sharing that the aforementioned doctor took down his video and apologized. As part of his update, Tsu Lange Yor’s creative director added, “No hard feelings from me.”
